ENTRY FORM INSTRUCTIONS
1. All entry forms must be completed by exhibitor and mailed to the following address
on or before September 1st.
Mail entries as soon as possible to:
Fair Secretary, Four Town Fair, P.O. Box 24, Somers, CT 06071

2. Use the entry form for specific animal or exhibit entered, or use general entry form
for all other exhibits.

3. DO NOT write in Award and Amount Column. For Secretary's use only.

4. One exhibitor per entry form, please.

5. Please read rules and regulations regarding time and location exhibits must be in place.

6. Parade Entry Forms send to: Parade Superintendent, P.O. Box 24, Somers, CT 06071-0024
